We
want to continue to offer opportunities for our youth
(under 18 years of age) to participate in mission trips
and other group field trips. If you are interested in
signing up for a planned activity, we encourage you
to discuss it with your parents.
Consent
Forms
A parental consent form will be required as part of
your registration. You may obtain a copy of the standard
consent form by printing it from this page. Parents
must fill out the form and sign it. The form, along
with any required fees, may be handed in to your youth
pastor, Christy Engelhart (Middle School) or Barry Valentine
(Senior High).
All checks can be made out to Oak Haven Church. Please
remember to make a note on the check for the name of
the event you wish to attend.
